Program Description
The Special Diabetes Program for Indians focuses on primary, secondary, and tertiary diabetic prevention and treatment and related data collection.

General Program Requirements
In order to qualify for this benefit program, you must be a Native American/American Indian and you or your family member must be enrolled in a federally recognized American Indian tribe or Alaskan Native village.
